Information and Application to Serve Form

As a member of the Birmingham South Cluster, we encourage you to prayerfully complete the Information and Application to Serve form. This form is used by the cluster to identify your areas of interest for service in the cluster. When there is a walk that involves the Birmingham South Cluster, the Lay Director should ask for and review this information to assist in planning a team.

Below is a review of the questions that are asked on the written form.  Please complete the Information and Application to Serve form and send to the Cluster Lay Director as noted on the form.

Please complete this service sheet whenever your information or changes, such as after serving on a team or at least once a year.

 

EXPLANATION OF QUESTIONAIRE FORM
 
The first section is basic information
  • We need your original walk number and community where your walk occurred.
  • The name of the church where you currently worship.
  • Your current involvement in a reunion group, a study or prayer group or another type of accountability group.
  • Clergypersons please note that you are clergy.

The second section pertains to team preparation for a walk and areas of team service in which you would be willing to serve

  • Have you attended a training event? Such as the annual training event by the Alabama Emmaus Community or another Community.
  • If you have attended a training event in the past, when and where?
  • Prior walk service-Have you ever served on a team?
  • If you have served on a team, when and where? Please identify if another community? Please note the committees in which you have served.
  • Team preparation. Are you able to make a commitment of 20+ hours of team preparation over a 6-8 week period of time? (This is a requirement of the Walk to Emmaus)
  • If no, please explain-please note your reason you are unable to complete the team preparation requirement.
  • The Alabama Emmaus Community Walk teams are divided into the following areas of service 
    • 72 Hour Prayer Vigil Committee
    • Agape Committee
    • Candlelight Committee
    • Clean-up Committee
    • Closing Committee
    • Conference Room Committee
    • Entertainment Committee
    • Follow-up Committee
    • Housing Committee
    • Meal Service Committee
    • Music Committee
      • Guitar
      • Piano
      • Voice
      • Other instruments
    • Refreshment Committee
    • Speaker’s Prayer Chapel
    • Sponsor’s Hour Committee
    • Worship Committee
 
The third section pertains to your desire to serve in the Birmingham South Cluster
  • If you would like to serve or assist with the music at cluster meetings
  • Provide refreshments for a walk when a need is identified 
  • Furnish or assist with agape for the cluster to provide for walks
  • Pray during the 72 hour prayer vigil. The cluster has time slots to fill for each walk
  • Serve on a cluster committee
